I Password disappears after opening a new tab in Firefox 12
I am a big fan of this software in general and use it every single day since quite some time.
But since Firefox 12, I have the very annoying problem that the plugin disappears as soon as I open a new tab. The button just disappears and the plugin is no longer able to fill out input fields. If I press "ctrl and /" the add-on bar will show up on the bottom of the browser window and if I click on the 1P icon the key-chain is locked (even if it was unlocked 20 sec. before). Even after unlocking, it is no longer able to fill out password fields in the browser. The plugin GUI also shows a red exclamation point icon, that says the 1Password Helper process is experiencing problems. After restarting Firefox it works as long as i use only one browser window.
Environment:
Windows 7 x64
Firefox 12 (Firefox profile is stored on another drive)
1P Plugin 3.9.4
1P 1.0.9.294

Thanks for taking the time to let us know about the trouble you're experiencing and for including the diagnostics reports. They really help a lot.
The lack of filling is likely due to the NoScript extension. Could you please disable the extension and restart Firefox? Then test to see if the filling works as it should.
The disappearing icon thing is a tad strange. Could you take a screenshot? Or better yet, if you can record a screencast of it happening I think that may help a lot. It doesn't need to be long, 10-15 seconds just to show what is happening. You can use Jing for this:

For the Add-on bar being locked, this is a known bug. It's related to trying to fix some memory issues with Firefox, hopefully it will be fixed in a future update.

Huge thanks for the video, very helpful.
We don't control the loading of the 1Password button, it's controlled by Firefox, so this is very tricky to diagnose since we also can't reproduce the issue either. I tried my best but I can't make the button disappear.
Usually, this is a misconfiguration issue in your Firefox Profile.
The quickest way to find out is to create a new Firefox Profile and see if 1Password works better in it. This guide from Mozilla will explain how to do this:
